Health form time!

Posted on Mar 24 2022 - 2:24pm

It's probably time to update your health form.  They are only good for 1 year.  I generally have mine done at my annual physical, and walk in with parts A & B completed.  For those going to summer camp, I need these in by the fourth of July (so I have time to check them, get them corrected, and get them copied).

Time to update health forms

Posted on Feb 26 2021 - 11:13am

Since we will be back to doing outings starting March 6th, we need to have updated Annual Health and Medical Records for ALL (adults and Scouts, registered or not) going on outings with us.  That way we know what we can do to help them if they get injured, and we have insurance information in case they need professional treatment.
